REGISTERED PSYCHOLOGIST
Per hour.	$135

EDUCATIONAL PSYCHOLOGIST
Psychometric testing, WISC IV and report with recommendations	$400
School liaison, Private tuition plans, Behavioural plans	$120 per hour.

COUNSELLING AND PSYCHOTHERAPY
Specialist Nurse Therapist	$80 - 120 per hour.
Therapist (CBT, Family Therapy, Child and Adult Psychotherapies)	$90 - 120 per hour

SPECIALIST PSYCHIATRIST
INITIAL ASSESSMENT Includes review of previous records / questionnaires, one hour clinical assessment / written summary & recommendations/initial prescribing if required.	$395
SCRIPT / FORM COMPLETION FEE	$25
PROFESSIONAL REPORT WRITING Information review, clinical assessment and writing if required in addition to summary and recommendations as above.	$250 per hour
REVIEW / FOLLOW UP	$125 per half hour
HOME / LOCATION VISITS ARE CHARGED BY HOURS OF APPOINTMENT TIME INCLUDING TRAVEL.

SPECIAL CONDITIONS

·Clients on low income can often be funded by WINZ or other agencies.

·Your family doctor can help you with this by writing a letter of support if he/she feels this is indicated.

·Payment can be by direct invoice to service purchasing agencies with prior written approval.

·Carer support days may be used for payment on approval of needs assessment agency.

·Flexible terms / sliding scale may be available by negotiation with individual clinicians.

·Starfish Specialist services are recognised for Health Insurance cover.

·Payment can be by instalment over period agreed between Starfish and clients.

·Receipt for payment will be provided on request.

METHOD OF PAYMENT

·Payment of all costs are required at time of appointment by cheque, EFTPOS or by prior bank transfer directly to the clinician providing services unless agreed otherwise.

·Direct transfer should clearly indicate your identity to enable account update.
