REGISTERED PSYCHOLOGIST
Per hour.	$150

EDUCATIONAL PSYCHOLOGIST
Psychometric testing, WISC IV and report with recommendations	$1,000
School liaison, Private tuition plans, Behavioural plans	$140 per hour.

COUNSELLING AND PSYCHOTHERAPY
Specialist Nurse Therapist	$80 - 120 per hour.
Therapist (CBT, Family Therapy, Child and Adult Psychotherapies)	$90 - 120 per hour

SPECIALIST PSYCHIATRIST
INITIAL ASSESSMENT Includes review of previous records / questionnaires, one hour clinical assessment / written summary & recommendations/initial prescribing if required. Payment plans accepted on request.	$425
SCRIPT / FORM COMPLETION FEE	$30
PROFESSIONAL REPORT WRITING Information review, clinical assessment and writing if required in addition to summary and recommendations as above.	$350 per hour incl GST
REVIEW / FOLLOW UP	$150
HOME / LOCATION VISITS ARE CHARGED BY HOURS OF APPOINTMENT TIME INCLUDING TRAVEL.

Dr Appleton is a registered specialist and 80% fees can be reclaimed from most medical insurance groups.

SPECIAL CONDITIONS

·Clients on low income can sometimes be funded by WINZ or other agencies. This must be arranged before any appointment with WINZ.

·Your family doctor can help you with this by writing a letter of support if he/she feels this is indicated.

·Payment can be by direct invoice to service purchasing agencies with prior written approval.

·Carer support days may sometimes be used for payment on 'EXEMPTION' by needs assessment agency. This must be approved in writing by Taikura Trust and MoH.

·Flexible terms / sliding scale / payment plans may be available by negotiation with individual clinicians.

·Starfish Specialist psychiatrist services are recognised for Health Insurance cover (80%). Please check with your provider regarding your own policy cover conditions.

·Payment can be by instalment over period agreed between Starfish and clients.

·Receipt for payment will be provided on request.

METHOD OF PAYMENT

·Payment of all costs are required at time of appointment by cheque, EFTPOS or by prior bank transfer directly to the clinician providing services unless agreed otherwise.

·Direct transfer should clearly indicate your identity to enable account update.
